    Dear Parent or Carer

When your child starts school your child’s health records are transferred to the School Nursing Team from the Health Visitor and they will remain within the School Nursing Team until your child reaches 19 years of age. The School Nursing Team can provide you with help, advice and support on many of the physical and emotional problems your child may encounter throughout their school life.

This year we are working with The Lancaster Model to offer an electronic questionnaire, each web-based questionnaire will take about 20 minutes to complete. You can find more information here https://www.thelancastermodel.co.uk/parents-guardians/

In line with government policy, the School Nursing service aims to involve families and young people in how resources are effectively targeted in the future.  In order to achieve this, please complete the online questionnaire about your child’s health and well-being within the next 7 days, this can be accessed here https://derby.hapi-system.com/school-entry or scan this QR code

Please enter the following location password – Hardwick2 and then select your child’s school from the list

Although anonymous information from the questionnaire may be shared with other agencies to improve services, all personal information collected will be treated as confidential.  A member of the school nursing team may contact you following completion of the questionnaire to offer health advice, information and support if indicated.

As your child progresses through their school career your child will be asked to complete a further health questionnaire in year 6 and again in year 9.  You will receive a letter informing you about this nearer the time. By completing the questionnaire you not only help us to plan future services, but also ensure that your child gets the health and well-being support they need.

If you need to contact the School Nursing Team please contact the Central Point of Access on the number above, contact chat health on 07507327754 or email: dmh-tr.ChildFamily-Derby@nhs.net.

    Yours sincerely –

  • Football Team Match Report vs St Albans

Wed 12 Oct 2022 Mr Gilbert

    St Albans 3 – 3 Borrow Wood

Borrow Wood took on St Albans in their first away match of the season. St Albans took an early lead but Borrow Wood replied soon after when a long-range strike from Aaron was palmed back into the area by the St Albans goalkeeper where Josh was lurking to tap in for the opener. The crossbar was rattled by Raife and Aaron twice in the following minutes. A Buddy strike following the interception of a goal kick gave Borrow Wood the lead at half time.

The start of the second half was dominated by the home side with them scoring an equaliser however Borrow Wood broke on the counter attack and Aaron slotted comfortably past the keeper to take the lead again. Borrow Wood continued to weather the storm of continuous attacks with Tapiwa, Douglas and Andreas all making last ditch tackles as well as a vital bloke by Joel. Xander’s quick reactions in goal meant he could pounce on any crosses and shots from close range. With ten minutes remaining the St Albans striker snuck in behind the defence capitalising on some miscommunication and calmly finished to take it to 3-3.

The last 5 minutes of the game was spent with Borrow Wood on the attack with several shots just missing the target and final passes intercepted by the St Albans defence. The final moment of the game saw Raife’s cross unfortunately avoid Josh and Buddy who had sprinted into the box, eventually going out for a throw in. The ref blew his whistle and signalled full-time in what was a thrilling, end-to-end, and extremely physical, 60 mins of football which both teams played in the right spirit. Borrow Wood are currently unbeaten with a win and a draw in their opening two matches scoring seven goals in the process. We now enter a winter break with the next fixture due to be played in March.

Man of the match went to Tapiwa for his energy to break up St Albans' attacks and set us up to attack ourselves in a performance where he covered every blade of grass on the pitch.

Please do not arrive too early for your appointment, it is important that we do what we can to avoid large groups congregating in the waiting area. It would also be helpful if you could avoid bringing children with you to your meeting.
